Dear Parent/Guardians:

 

Please be reminded that in order for your child to participate in fall sports several items need to be completed and submitted. This includes FamilyID for fall sports registrations and all required medical forms (HHQ/PPE). 

 

Attached are directions to sign up for FamilyID, as well as how to upload medical forms electronically to Genesis.   We utilize FamilyID for portions of our athletic registrations. A PPE form and Health History Questionnaire (HHQ) also need to be completed for athletic clearance and participation.

 

All medical forms and FamilyID registrations must be completed by July 18, 2025. Please be reminded we no longer accept or process PAPER forms; all forms are to be submitted electronically.

  

Reminders:

The due date for all FALL athletic registrations and paperwork is Friday, July 18th; please submit ASAP - the sooner the better!

  • As a part of this process, BE SURE YOU HAVE AN UPDATED PHYSICAL on file. Physicals are good for one calendar year – be sure it is updated in order to be eligible to compete in athletics this school year.
  • All prospective fall athletes MUST submit a HHQ/MEF form, regardless of whether they played a spring sport or not. Physicals are an ANNUAL requirement; HHQ form is a SEASONAL requirement. You have to submit one even if you did for the winter/spring seasons.
  • All required forms are on the school website and can also be found on the NHRHS FamilyID website. Click the link to access: Required Forms. Please review the Athletic Information Packet for all requirements, tryout information, dates, deadlines etc.
