Dear Parent/Guardian(s):
We trust this finds you well as we close out another school year. In preparation for the Fall season, please be aware of several items that require your attention and cooperation for your child to participate.
Please know you can start submitting all required medical forms. Families will complete an updated Preparticipation Physical Evaluation (PPE) form and Health History Questionnaire (HHQ) for athletic clearance and participation, healthcare professionals will need to complete the PPE Medical Examination Form (MEF) & Medical Form B. Upload ONLY the HHQ, MEF & Medical Form B to Genesis (the PPE will remain on file in your healthcare provider’s office). All forms, as well as instructions, are listed on our website for your convenience: NH Athletics Required Medical Forms .
In addition, we have launched FamilyID for Fall sports registrations. We utilize FamilyID for portions of our athletic registrations - attached are directions to sign up for FamilyID.
All medical forms and FamilyID registrations must be completed by July 18, 2025. Please be reminded we no longer accept or process PAPER forms; all forms are to be submitted electronically - instructions on how to do so are attached.
Please review the summary of tasks listed below in order to gain medical clearance:
- Complete the PPE form (Annual Physical Form). This Preparticipation Physical Exam form should remain with your Healthcare Provider after your physical exam is complete; DO NOT SUBMIT THIS FORM TO THE SCHOOL. This needs to be completed annually.
- Complete and submit the PPE Medical Eligibility Form (MEF). This should be completed by your Healthcare Provider during your physical exam then uploaded to Genesis Parent Portal.
- Complete and submit the Health History Questionnaire (HHQ). This is a SEASONAL form that must be uploaded to Genesis for each sport in which your child participates.
- Medical Form B (NH Medical Form). This should be completed by your Healthcare Provider during your physical exam then uploaded to Genesis Parent Portal. Please know this form is voluntary - if you do not submit this form your child will be tested by the nursing staff upon their return to school.
- Register in Family ID. This must be completed for each SEASON of participation and can be found in the link above to the Northern Highlands website.
Here are some additional reminders:
- The due date for all FALL athletic registrations and paperwork is JULY 18th; please submit ASAP - the sooner the better! For full summer workout clearance please complete this process ASAP (only a HHQ is required; head coaches have made student athletes aware of this).
- BE SURE YOU HAVE AN UPDATED PHYSICAL on file. Physicals are good for one calendar year – be sure it is updated in order to be eligible to compete in athletics next school year.
- Please know Winter and Spring sport registrations do not take place until approximately 60 days prior to the start of the respective season.
