Dear Parents and Guardians: To access medical forms that pertain to your child , please click on "Forms" tab to the left. This year, I will be screening tenth graders for height, weight, vision, and hearing. You will be notified of any problems or referrals. In addition, I will be checking your child for scoliosis, which is a curvature of the spine. If your child is under the care of a doctor for this condition, or if you do not want your child screened, please notify me, in writing, by October 9, 2011. A vital part of our school health services requires our having up-to-date records on your child. If your child receives an immunization of any kind, please send me a note so I can keep the health records current. If your child is ill, please call the attendance office at (201) 327-8700 ext. 260 on the first day of absence. Any contagious illness requires a doctor’s note upon returning. Medication may be given to students only under certain conditions and with a doctor’s note. (See page seven of the Student/Parent Handbook.) To access necessary medical forms, including life-threatening allergies requiring Epipen or illness such as asthma, click on the "Forms" menu to the left. Open, print, and complete forms, as needed.
